Job summary

CareRing Health in Maryland is seeking a compassionate Licensed Practical Nurse to join our home-based care team. You will conduct patient home visits, assess needs, and coordinate with physicians, nurses, therapists and social workers to deliver comprehensive care in patients’ homes.

Responsibilities include managing care plans, administering medications, handling emergencies, and preparing timely progress notes in compliance with regulations.

Qualifications

• Active LPN state license.
• Valid driver's license.
• Two or more years of nursing experience.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated home care; conduct patient home visits.
• Assess, plan, direct, and evaluate each patient’s care plan to meet needs.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care in accordance with applicable regulations and company policy.
• Prepare and maintain accurate clinical and progress notes in a timely manner.
