Job summary

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to transportation.
• CPR certification or ability to obtain within 90 days.

Responsibilities

• Provide high quality clinical services within the scope of practice as defined by state laws.
• Document patient visits thoroughly in the electronic medical record.
• Coordinate care activities and communicate with patients, families, physicians, and the care team.
• Function effectively in diverse home environments and adapt to patient needs.
