Job summary

Professional Case Management in New Mexico is hiring LPNs/LVNs to provide in-home healthcare services and build one-on-one patient relationships. You control your schedule to fit school, travel, volunteering, or family time.

Ideal candidates have nursing education and licensure in good standing, with CPR and TB testing. Acute care experience is preferred but new grads are considered; standard rates apply for in-home care roles.

Qualifications

• Graduate of a state-approved school of professional nursing.
• Current, unrestricted LPN/LVN license in the state(s) of practice.
• Prefer minimum of one year experience as an LPN/LVN in an acute care setting or equivalent; will consider new graduate nurses.
• Current CPR certification and TB test.

Responsibilities

• Provide in-home healthcare services to help clients in their homes.
• Engage in one-on-one patient relationships and monitor health status.
