Job summary

UnitedHealth Group seeks an experienced Home Health LPN/LVN to provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse. You will verify plans of care, collaborate with physicians and RNs, and educate patients and caregivers in self-care during visits.

The role requires 1+ year LPN/LVN experience, an active LPN license for the assigned state(s), CPR certification, and a valid driver's license.

Qualifications

• LPN/LVN clinical experience required.
• Current LPN license, valid for assigned state(s).
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
