Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) to provide compassionate in-home patient care. You will follow the prescribed plan of care, monitor patients, and document vitals and progress while administering medications and IV fluids as needed.

The role emphasizes one-on-one care, collaboration with clinicians, and a commitment to dignity and comfort for clients in their homes. Flexible FT/PT/PRN schedules are available to support work-life balance.

Qualifications

Graduate of a practical nursing program (LPN/LVN) with current licensure in the state where the position is located.
Reliable transportation to/from care sites and/or work locations.
Practical trach and/or ventilator experience preferred, not required.

Responsibilities

Provide in-home direct patient care according to the plan of care and nursing scope.
Assess and document patient conditions and report changes as needed.
Administer medication, insulin, and IV/fluids with thorough documentation.
Inspect and care for wounds, change dressings, assist with personal hygiene.
Assess for further injuries or infections and deliver appropriate treatments.
Collaborate with providers and other health professionals to ensure quality outcomes.
