Job summary

UnitedHealth Group in Spokane, WA seeks a Licensed Practical Nurse in Home Health to provide high-quality patient care in the patient’s home, following the plan of care and infection control procedures.

Responsibilities include documenting visits in the electronic medical record, coordinating with the care team, and delivering care in diverse home settings. Required WA LPN license, valid driver's license, and strong communication skills; CPR preferred; full-time with comprehensive benefits.

Qualifications

• Current and unrestricted LPN licensure in Washington state.
• Valid driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
• CPR certification preferred; home care experience preferred.
• Ability to manage multiple tasks and work independently.
• Strong communication, writing and organizational skills.

Responsibilities

• Provides high quality clinical services within the scope of practice per state laws and plan of care.
• Documents patient visits thoroughly in the EMR and complies with regulatory requirements.
• Participates in care coordination, communicates needs and changes with the team and family.
• Able to work in diverse home situations regardless of patient demographics.
