Pathfinder, Inc. seeks a Supportive Living Companion to provide tailored services in the consumer's home and community, supporting self-help and socialization goals. You will maintain logs, report incidents, and ensure compliance with policies and background checks.

The role includes training in independent living skills, home management, cooking, cleaning, and supported employment activities, with scheduling on Saturdays and Sundays. A CNA or Medical Assistant background may qualify for entry.

Qualificações

• High School diploma or GED is required.
• 1 year of relevant supervised work experience in public health, human services, or community service.
• 2 years' experience with individuals with developmental disabilities.

Responsabilidades

• Provides services within consumer's home and community per person-centered plan.
• Trains and supports individuals in independent living, home management, cooking, cleaning, and socialization.
• Maintains incident reports and logs per DHS policy and plan objectives.
• Ensures medication reminders when needed and completes required documents.
