Job summary

Yale NewHaven Health seeks a Licensed Practical Nurse for home health services in Connecticut. The role requires assessing patients during each home visit, administering treatments per physician orders, and educating patients and caregivers as outlined in the care plan.

Ideal candidates hold an approved LPN program, CT licensure, CPR certification, and a valid driver’s license with car insurance. A minimum of one year in acute care is preferred, with readiness to travel to multiple stops daily.

Qualifications

• Graduated from an approved Practical Nurses Education Program.
• Minimum one year experience in the acute care setting; home care experience preferred.
• LPN license with valid Connecticut license; CPR certification required; valid driver’s license and proof of car insurance for personal vehicle use.
• Candidates must obtain CPR certification within 3 months (90 days) of hire.

Responsibilities

• Assesses patients within scope of practice at every home visit.
• Carries out treatments per MD orders and reports to RN case manager or clinical manager.
• Reports all changes in condition to the MD.
• Provides patient/caregiver education as per care plan.
• Completes all documentation within 24 hours with changes in condition and follow-up details.
• Attends all staff meetings and mandatory trainings.
• Follows policies and procedures and mandatory requirements including Healthstreams.
• Maintains and supports high quality patient care and safety.
