Northeast Healthcare Recruitment seeks experienced Nurse Practitioners to perform in-home Health Risk Assessments for patients across Clackamas County, Oregon. This flexible, part-time role lets you set your own hours with visits available seven days a week, and includes mileage reimbursement and no-show compensation.

Applicants must hold an active Oregon NP license and have strong clinical assessment and documentation skills; reliable transportation is required for in-home visits.

Competenze

• Active Nurse Practitioner license in Oregon.
• Experience with Health Risk Assessments (HRA) preferred.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage schedule efficiently.
• Reliable transportation for in-home visits.

Mansioni

• Conduct comprehensive in-home Health Risk Assessments (HRAs)
• Identify and document acute and chronic medical conditions
• Obtain vital signs and calculate BMI
• Review medications and pharmacological therapy
• Perform a physical examination
• Evaluate patients’ physical, mental, and social health needs
• Educate and counsel patients and their families on health conditions and preventative care
