The position is a pure assessment role with no prescribing, no on-call, and no follow-up care. Flexible scheduling and autonomy are emphasized, with mileage reimbursement after 20 miles/day and rapid onboarding.

Qualifications

• Active NP license (no PAs; exclusions listed)
• Reliable personal transportation to cover multiple counties
• Able to start ASAP

Responsibilities

• Conduct in-home Health Risk Assessments (HRAs)
