Job summary

UnitedHealth Group is seeking a Full-time Hospice Social Worker for Grant Memorial Hospice in Petersburg, WV. The role provides in-home social work services under physician guidance and an interdisciplinary team to address social and emotional factors affecting patients’ health.

The position requires a Master’s Degree in Social Work, WV licensure, a valid driver’s license, and travel within Grant, Hardy, Hampshire and Pendleton counties. Evening/weekend hours may be required.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Licensed as a Social Worker by the State Board of Social Work in WV

Responsibilities

• Assists patients and families, physician and care team to understand social and emotional factors related to health status
• Contributes to the development of a comprehensive Plan of Care for patients on a daily basis
• Instructs care team on community resources to assist patients as needed
