Job summary

Grant Memorial Hospice, part of the Optum family, seeks a dedicated Hospice Social Worker to provide in-home social work services under physician direction. The role focuses on significant social and emotional factors affecting patient health and coping strategies within a multidisciplinary team.

The position requires an MSW, WV social work license, a valid driver's license, and willingness to travel within Grant, Hardy, Hampshire and Pendleton counties.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• Current Driver's License and access to a dependable vehicle or public transport.
• Licensed as a Social Worker by the State Board of Social Work.

Responsibilities

• Assist patients and families to understand social factors related to health.
• Contribute to a comprehensive Plan of Care with the health care team.
• Inform the team about community resources available to patients.

As the Social Worker, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient’s health status and in development of coping mechanisms.

Grant Memorial Hospice in Petersburg, WV is seeking a Full Time Hospice Social Worker.

• Full‑time position providing in‑home visits for hospice patients.
• Master's Degree and Social Work license in WV is required.
• Standard business hours are Monday-Friday, 8:00 AM‑4:30 PM.
• Occasional evening/weekend hours as needed.
• Travel required within a Grant, Hardy, Hampshire and Pendleton counties to provide patient care.

Primary Responsibilities:

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient’s health status on an as needed basis.
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
• Instructs health care team members on community resources available to assist patients on a as needed basis.

Required Qualifications:

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Licensed as a Social Worker from the State Board of Social Work

Preferred Qualifications:

• 1+ years of social work experience in a health care setting
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
