Job summary

AdamsPlace seeks a Chaplain PRN to provide in-home spiritual care to hospice patients and families. The role involves assessing spiritual needs, coordinating with area clergy, and supporting bereavement processes. Strong listening and interfaith sensitivity are essential, with on-call availability after hours.

Requires a graduate theology degree (M.Div. preferred) and at least 1 year of hospice chaplaincy experience.

Qualifications

• Graduate degree in theology, divinity or pastoral counseling (M.Div. preferred).
• At least 1 year of chaplaincy experience in healthcare preferred.
• Comfort working in an interfaith setting with diverse beliefs and cultures.

Responsibilities

• Contacts patient/caregiver within five days of admission to hospice for an initial spiritual assessment.
• Document ongoing assessments, interventions and outcomes in the Electronic Medical Record.
• Assist IDT in understanding religious issues related to hospice care and bereavement.
• Coordinate and organize home visits efficiently.
• Engage community ministers to meet diverse spiritual needs across denominations.
• Communicate with and support patient’s caregiver clergy as appropriate.
• Provide consultation and education on spiritual concerns affecting end-of-life care.
• Assist with bereavement calls to caregivers after death.
• Utilize the EMR for visit documentation on the day of the visit.
• Be willing to participate in on-call rotation for after-hours visits as needed.
• Support volunteers and participate in volunteer training when appropriate.
