Northshore Home Health Care is seeking a dedicated In-Home Healthcare Registered Nurse to deliver high-quality patient care in home settings across Texas.

You will perform comprehensive assessments, administer medications, develop care plans with the care team, educate families on disease management and safety, and document all findings in an EMR. The role emphasizes compassionate, patient-centered care and strong communication with patients and families.

Qualifications

• Current RN licensure is required.
• Ability to assess patient needs and implement individualized care plans.
• Proficiency with medication/treatment administration.
• Excellent communication and patient/family education skills.
• Strong documentation skills in an electronic medical record.

Responsabilités

• Perform comprehensive patient assessments and monitor clinical status.
