Job summary

Health Link Home Health & Hospice seeks a compassionate Home Health RN to join our San Francisco team as Case Manager. You will perform comprehensive nursing assessments and coordinate care with physicians and the interdisciplinary team to support patients at home.

You will administer medications, provide wound care (including Wound Vacs), and document with company-issued iPads. A CA RN license and 1 year of experience are required, with training available for hospital/SNF nurses.

Qualifications

• CA RN license required.
• 1 year of home health or hospital/SNF nursing experience preferred.
• Wound care including Wound Vac experience a plus.
• Documentation on company-issued iPads.
