Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Sardis, MS. This role offers a flexible schedule and the chance to make meaningful connections with patients in their homes, rather than a clinic setting.

Qualified NPs will conduct comprehensive assessments, review medical histories, and document findings accurately while traveling to patients’ homes.

Qualifications

• Masters Degree in Nursing or commensurate experience with NP license.
• Active State NP license required.
• National board certification required.

Responsibilities

• Travel to patients' homes to conduct HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately and timely.
• Conduct medical reconciliation for patient safety and compliance.
• Provide basic patient education on wellness and preventive care.
