Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Muncie, IN. This opportunity offers flexible scheduling and the freedom to achieve professional goals on your terms, with travel to patients’ homes and comprehensive home assessment training.

Nurse Practitioners must hold an active NP license, national board certification, and current BLS certification.

Qualifications

• Master's degree in nursing or commensurate experience.
• Active NP license in the state of practice.
• Active national board certification for NP.
• Current BLS certification.
• At least 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventive care.
