Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Kokomo, IN. This opportunity offers a flexible schedule, travel reimbursement, and comprehensive training.

NPs interact with patients in their homes, performing non-invasive physical assessments and documenting findings to support preventive care. Qualified candidates include Masters or equivalent experience, active NP license, board certification, and

Qualifications

• Masters Degree in Nursing or commensurate experience.
• Active NP license in state of practice.
• Active national board certification (NP-C) and BLS.

Responsibilities

• Travel to patients' homes and perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic wellness education and preventative care guidance.
