Job summary

Hueman is hiring Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members in and around Evansville, IN. This unique role adapts to your lifestyle, offering a flexible schedule and the chance to serve patients in their homes.

You will travel to patients’ homes, conduct non-invasive assessments, review histories, document findings, and provide basic wellness education. Eligible candidates hold a Master’s in Nursing, an active NP license, board certification,

Qualifications

• Master’s Degree in Nursing (or commensurate experience) and completion of NP license.
• Active State NP license.
• Active National Board Certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Education

Master’s degree in nursing

Active State NP license

Active National board certification

Current BLS certification
