Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Bastrop, TX. Responsibilities include travel to homes, comprehensive non-invasive assessments, medical history review, accurate documentation, and basic wellness education.

Qualifications

• Master’s degree in Nursing or commensurate experience.
• Active State NP license and national board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings.
• Conduct medical reconciliation for safety and compliance.
• Provide basic education on wellness and prevention.
