Qualifications

• Active CA Therapy License.
• 1+ years of licensed experience.
• Valid driver's license, reliable transit, and auto insurance.
• Current CPR certification.
• Professional liability Insurance.
• Recent physical exam (within 2 years) and TB clearance.
• Active EIN and business license/city tax certificate.

Responsibilities
