Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist Assistant to help patients regain strength and independence in their homes. You will implement therapy plans, perform manual therapies, document progress, and collaborate with the clinical team to ensure high-quality care.

Requirements include an active PTA license in good standing, valid driver's license with reliable transportation, CPR certification, and fluency in English. Prior geriatric care experience is valued.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric population changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control and universal precautions in patient care activities.
