Job summary

Matrix Medical Network offers NP-led in-home health assessments across the nation, reviewing health history, medication adherence, and social factors to close care gaps. Visits may last up to one hour and are conducted at members’ residences, including homes and SNFs.

Providers perform health assessments, order tests, and work with physicians and care teams to improve outcomes. Flexible scheduling and competitive pay are emphasized in the role.

Qualifications

• Must hold NP licensure in state of practice and prescriptive authority if required.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license and auto insurance for the state of residence.
• Strong computer skills and experience with health/medical record software.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, family members, and other staff.
• Provide services at member homes or Skilled Nursing Facilities.
• Document care coordination and follow-up per Matrix guidelines.
