Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, visiting members in their homes and bridging gaps in care. The role emphasizes connecting patients to the right care and requires adaptable travel within licensed states.

Visits last about 45 minutes, include medication and medical history review, a physical evaluation, and optional tests.

Qualifications

• Active, unrestricted NP/PA license in coverage area SEATTLE SNOHOMISH.
• Board certification required.
• Multi-state licenses preferred.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role

• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
