Qualifications

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other team members regarding patient needs and status changes.
• Participate in care coordination activities and discharge planning.
