Humana Inc. through CenterWell Home Health is seeking a Home Health LPN to provide one-on-one, in-home nursing care and implement individualized care plans under RN supervision. You will educate families, document care, and participate in discharge planning with the health team.

The role requires an active LPN/LVN license, CPR, a valid driver's license, and 2 years of clinical LPN/LVN experience. Schedule is 40 hours per week with a pay range of $58,300–$80,300 per year.

Formación

• Active LPN/LVN license in the practicing state.
• Valid driver’s license with reliable transportation.
• Current CPR certification.
• Minimum 2 years of clinical LPN/LVN experience.

Responsabilidades

• Provide skilled nursing care to patients in their homes under RN supervision.
• Implement individualized care plans with patient, family, and healthcare team.
• Educate patients and families on disease management and self-care techniques.
• Maintain accurate documentation of patient visits, interventions and outcomes.
• Communicate with patients, physicians and care team about patient status.

Conocimientos
