Job summary

Humana Inc. through CenterWell Home Health is seeking an LPN to provide in-home nursing care, coordinate with physicians, and support patient recovery.

You will work under an RN, implementing tailored plans and educating families, with emphasis on safety, accuracy, and timely communication. The role requires an active state LPN/LVN license, CPR, a valid drivers license, and at least two years of clinical LPN/LVN experience.

Qualifications

• Current nursing license in the practicing state.
• Valid drivers license, auto insurance and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and their families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.
