Job summary

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. In this field-based role, you’ll conduct histories, exams, risk assessments, and evaluate social determinants that impact care.

You will document findings in EMR systems, educate patients and caregivers, and collaborate with physicians and health plans to improve outcomes while delivering flexible, patient-centered care.

Qualifications

• Must be a licensed Nurse Practitioner or Physician Assistant.
