Job summary

FOX Rehabilitation is expanding in West Bend, WI, seeking licensed Physical Therapists to deliver one-on-one geriatric care in private residences or senior living communities. Clinicians have autonomy to manage their schedules and may incorporate telehealth for flexible, continuous care.

Responsibilities include evaluations, individualized care plans, and evidence-based treatment, with collaboration across PT, OT, SLP, and fitness teams.

Qualifications

• Licensed Physical Therapist in WI or eligible to obtain license.
• DPT or PT degree from an accredited program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in home or senior living settings.
• Develop individualized plans of care and deliver evidence-based treatment.
• Deliver sessions to support mobility and independence.
• Document in the EMR per regulatory standards.
• Collaborate with an interdisciplinary team.
