FOX Rehabilitation is growing in Emmaus, PA, and seeks licensed Physical Therapists to deliver one-on-one care to older adults in homes or senior living settings. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes through the Geriatric House Calls model.

Clinicians may include telehealth and virtual care to extend access while collaborating with an interdisciplinary team and maintaining high standards of documentation and care delivery.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program (DPT preferred).
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access and flexibility.
