Job summary

WellBe Senior Medical is seeking a Patient Care Advocate II (LPN/LVN) to deliver high-quality in-home care to frail, Medicare Advantage patients. You will coordinate visits, facilitate telehealth with APP/Physician, document data, and educate patients.

You're part of an interdisciplinary team, travel locally, and operate within LPN scope under supervision. This role emphasizes patient advocacy, care coordination, and adherence to regulatory standards while supporting quality measures.

Qualifications

• High-school diploma or GED required
• Active, unrestricted LPN/LVN license with residency in the state of service
• Minimum of two years as an LPN/LVN in a direct patient-facing role
• Current BLS/CPR certification for healthcare providers
• Ability to perform venipuncture and capillary draws where required by market
• Valid driver's license and auto-insurance
• Prior telehealth experience preferred
• Experience in home care, hospice, or geriatrics encouraged

Responsibilities

• Facilitate in-home telehealth visits between the WellBe APP/Physician and the patient
• Record vitals and document health changes
• Conduct home-safety inspections during visits
• Provide direct patient-care support within LPN scope
• Perform POCT and explain results to geriatric patients
• Safely perform venipuncture and IV therapy where allowed
• Support initial, follow-up, and urgent visits; engage disengaged patients
• Escalate clinical concerns to APP or Medical Leadership
• Coordinate care and education for patients and families
• Maintain comprehensive patient documentation in EMR
• Participate in IDT meetings and team collaboration
• Build local-provider relationships and represent WellBe in markets
