Acenda, Inc. is seeking a Family Support Worker to build trusting relationships with families through in-home visits, typically lasting about an hour. The role focuses on strengthening parent-child relationships and delivering strength-based interventions within an integrated health framework.

The candidate should be adept at screening, assessments, goal setting, and coordinating with health and social services. Some evening hours and NJ-wide travel may be required.

Formación

• Minimum of a High School Diploma or GED with two years of experience with children (0-3) and families.
• Knowledge of infant and child development and normal growth.
• Experience working with community agencies and referrals.

Responsabilidades

• Outreach to families referred through Connecting NJ or service provider.
• Use screening tool to determine eligibility for home visitation services.
• Interview using FROG Scale to identify strengths and risks.
• Strengthen parent-child relationships and support home visitation goals.
• Refer to other supportive agencies and coordinate healthcare appointments.
• Perform other related duties as assigned.
