Job summary

Haven Focused, LLC is an Indianapolis-based agency seeking Home Based Family Caseworkers to provide family-centered services to children and families involved with Indiana DCS and Probation. Services focus on keeping children safely at home, preventing foster care placement, and promoting family reunification.

Responsibilities include face-to-face contact within 48 hours of referral, comprehensive intake within 30 days, and ongoing plan reviews every 30–90 days, with court appearances as needed.

Qualifications

• Bachelor’s or Master’s degree from an accredited university (or High School/GED/Associate with 4 years of direct service experience)
• Strong written and verbal communication skills; basic computer skills for documentation
• Ability to work independently and manage time effectively, with a flexible schedule

Responsibilities

• Make face-to-face contact with referred families within 48 hours of referral
• Complete comprehensive intake assessments within 30 days
• Develop and implement client-centered treatment plans with measurable outcomes
• Review plans with families every 30 days and reassess every 90 days
• Write daily progress notes and monthly progress reports (by the 10th of the following month)
• Provide home visits and parent education in the family’s home
• Support reunification and family stability through in-home services and court appearances
• Coordinate services with DCS, Probation, and treatment teams and attend Family Meetings
