Job summary

Center for Family Services in East Orange, NJ is seeking an Outreach Worker for the Passaic County FPS team to prevent unnecessary out-of-home placements and strengthen family functioning through community linkages and services.

The role involves managing a caseload of 2–3 families, delivering in-home counseling 5–10 hours weekly, developing treatment plans, coordinating care with stakeholders, and maintaining detailed records; evening/weekend hours and travel are required.

Qualifications

• Bachelors required in human services or related field and at least two years' experience working with families, providing in home counseling and parent/family support.
• Demonstrates knowledge and experience in care coordination for families and community stakeholders.
• Embedded in the communities that are being served.
• Current driver's license with a good driving record of at least three years.
• Vehicle with current insurance and registration.

Responsibilities

• Maintain a caseload of 2-3 families and provide direct services for 5–10 hours weekly.
• Develop treatment plans after strength-based assessments.
• Implement a full range of interventions using individual, family, and group methods.
• Advocate for and provide concrete goods and services to meet family goals.
• Support FPS model with on-call availability 24/7 on a rotating weekend schedule.
