Qualifications

• Bachelor's in Social Work or related field is required.
• Valid Driver's License and car insurance are required or preferred for community-based work.
• Strong communication and collaboration skills are essential.

Responsibilities

• Establish initial contact with families within 24 hours of referral.
• Develop and implement Safety Plans and Family Action Plans.
• Coordinate care with DCBS and providers, ensuring timely documentation.
• Facilitate team meetings and ongoing communication with families and partners.
• Travel regionally for home visits and trainings as needed.
