• Valid driver's license and car insurance required.

Responsibilities

• Establish initial contact with families within 24 hours of referral.
• Initiate safety planning and connect families with resources.
• Coordinate with DCBS, providers, and family to implement action plans.
• Maintain documentation and communicate progress to DCBS and family.
• Travel regionally for home visits and trainings.
