Job summary

PACE Detroit seeks a Direct Home Care Worker to provide in-home and community support for participants, including personal care, meal prep, transportation, and accompanying to appointments. You will document visits accurately, report deviations to supervisors, and maintain safety while complying with CMS-approved competencies and infection control requirements.

A high school diploma or GED, 2 years of relevant experience, and completion of a Direct Care/Nurse Assistant training are required; a

Qualifications

• Minimum High School Diploma or GED.
• Must have minimum of 2 years of relevant work experience.
• Completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.

Responsibilities

• Personal care and assistance with activities of daily living.
• Assistance with meal preparation and toileting/incontinence support.
• Transportation and off-site appointment support.
• Accurate and timely documentation of care and progress; report to RNCM and supervisor.
