Job summary

Herewith is seeking compassionate Care Partners to support dialysis patients at home across Chicago. You will help with setup, safety, and daily wellbeing, working closely with a clinical team to deliver patient-centered care.

Role emphasizes reliability, empathy, and strong interpersonal skills. Training and placement assistance are provided, with consistent weekly hours and a stable care plan tailored to each client.

Qualifications

• Minimum 1 year of hands-on dialysis experience.
• Experience supporting dialysis patients (home or clinical).
• Familiarity with home dialysis machines preferred.
• Reliable, empathetic, and detail-oriented with strong communication.
• Must pass background check and vetting via Herewith.

Responsibilities

• Set up and support dialysis treatments at patients' homes.
• Provide companionship, emotional reassurance, and ensure schedule adherence.
• Monitor safety and assist with ADLs as needed.
• Maintain communication with clinic nurses and care coordinators.
• Document concerns or changes observed during support hours.
