Job summary

Adaptive Home Health, LLC is seeking a Certified Occupational Therapy Assistant (COTA) to provide skilled visits in patients' homes under an OT plan of care. This field-based, full-time role supports a high-acuity home health model in the Denver area.

You will deliver interventions for ADLs/IADLs, safety, energy conservation, and UE function while coordinating with nursing and the broader care team. A Denver COTA license and daily travel across the territory are required.

Qualifications

• Must have strong clinical judgment and ability to work independently in the field under OT supervision.
• Excellent communication and thorough documentation skills.
• Willingness and ability to travel daily across the assigned territory.

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and the broader care team: escalates changes in condition and safety concerns.
• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
