Job summary

Quality Healthcare Group, INC is seeking a Cognitive Therapist to assess, develop, and implement cognitive rehab programs for patients receiving in-home healthcare services in the Philadelphia area. The role emphasizes improving memory, attention, problem solving, and executive function while coordinating with nurses and other therapists.

You will document progress, ensure HIPAA compliance, and participate in ongoing professional development.

Qualifications

• Master’s degree or higher in a related field with cognitive rehabilitation training
• Active Pennsylvania licensure/certification as applicable (e.g., CCC-SLP, OTR/L)

Responsibilities

• Conduct comprehensive cognitive and functional assessments to establish baselines and identify deficits
• Develop individualized treatment plans considering patient goals and cultural factors
• Provide direct in-home therapy to improve attention, memory, reasoning, planning, and executive function
• Use evidence-based interventions and assistive technologies to support daily living and independence
• Monitor patient progress and adjust plans as needed
• Collaborate with nurses, caregivers, and other therapists to integrate cognitive goals into overall care
• Educate patients and families on cognitive strategies to sustain improvements beyond sessions
• Maintain HIPAA-compliant documentation and support audits
