Job summary

No Place Like Home Health LLC is seeking a motivated Home Health Aide CNA to join the team in Southfield. You will provide essential personal care and support to clients in their homes, ensuring safety, comfort, and well-being.

Your responsibilities include assisting with ADLs, monitoring health, medication reminders, and safe transfers. You will document care in EMR systems while upholding HIPAA compliance and offering compassionate, patient-centered service.

Qualifications

Certified Nursing Assistant (CNA) certification required.
Experience in home health or senior care preferred.
Knowledge of medical terminology and patient safety protocols.
CPR certification required; additional certifications are a plus.
Ability to work independently with a compassionate, patient-centered approach.

Responsibilities

Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility.
Monitor health by observing vital signs and reporting changes.
Provide medication reminders and assist with administration per protocol.
Support mobility with aids and safe transfers.
Document care activities in EHRs while maintaining HIPAA compliance.
Assist with light housekeeping, meal prep, and safe home environment.
Observe and report changes in mental status related to dementia or Alzheimer's care.
