Responsibilities

• Assist clients with ADLs (bathing, dressing, grooming, mobility).
• Monitor patient health by observing vital signs and reporting changes.
• Provide medication reminders and assist with medication administration.
• Support clients with mobility aids and safe transfers.
• Document care activities in EMR while maintaining HIPAA compliance.
• Assist with light housekeeping, meals, and safe home environment.
• Observe and report changes in mental status related to dementia or Alzheimer’s care.
