Job summary

Elite Home Health & Hospice is seeking compassionate CNAs to join our home health team. You will assist patients with daily living activities, provide respectful care in patients’ homes, and follow care plans to ensure safety and comfort under nursing supervision.

You will communicate changes in patient condition, work collaboratively with a multidisciplinary team, and maintain high standards of compassionate care in a home setting—all while supporting CAPLICO values and patient-first service.

Qualifications

• Active CNA certification (state-licensed).
• Valid driver’s license with reliable transportation.
• Strong communication skills and a team-oriented mindset.
• Ability to work independently in home settings.
• Commitment to patient-focused care.

Responsibilities

• Assist patients with activities of daily living (bathing, hygiene, dressing, mobility).
• Provide respectful, compassionate care in patients’ homes.
• Observe and report changes in patient condition to nursing team.
• Follow individualized care plans and safety guidelines.
• Collaborate with nurses and interdisciplinary team members.
