You will work with families and care teams to implement individualized plans, document progress, and participate in ongoing training to stay current with best practices in home care.

Qualifications

• 1 year of experience in healthcare or caregiving preferred.
• High School Diploma or equivalent is required.
• Excellent communication and interpersonal skills are essential.
• Ability to lift 25 pounds, stand for long periods, and perform bending/reaching tasks.
• Valid driver's license and reliable transportation (if applicable).
• Flexibility and adaptability for varied shifts including evenings/weekends.

Responsibilities

• Provide individualized in-home care, including daily living activities, medication management, and personal care.
• Build strong relationships with clients, families, and caregivers for clear communication and care coordination.
• Collaborate with care teams to develop and implement care plans meeting client needs.
• Maintain a safe and clean home environment, manage medications, and address issues promptly.
• Document client records and reports and communicate updates to teams and families.
• Participate in ongoing training to stay current with best practices.
