• Assist patients with personal care activities such as hygiene, mobility, and elimination as prescribed. Report any inability to perform tasks to the RN or Director of Clinical Services immediately.
• Create and maintain a safe environment for patients, using equipment properly (footstools, side rails, oxygen, etc.).
• Follow Universal Infection Control Precautions at all times.
• Help with home maintenance activities like light housekeeping and meal preparation as per the care plan.
• Monitor and document patient’s fluid and nutrition intake, vital signs, skin condition, and changes in body functioning.
• Practice safe transfer, ambulation, positioning, and range of motion techniques.
• Respect patient confidentiality and autonomy.
• Accurately document care provided and patient/family responses, submitting reports promptly.
• Operate within your training and experience limits.
• Ensure compliance with all applicable laws, regulations, and agency policies.
• Manage caseload efficiently and deliver cost-effective care.
• Meet or exceed the agency’s service standards consistently.
• Conduct all activities professionally and ethically.
• Use supplies and equipment efficiently.
• Complete at least 12 hours of in-service training annually.
• Maintain regular attendance and adhere to the Code of Conduct.
• Engage positively with staff, supporting the agency’s mission and vision.
• Follow orientation and policies related to confidentiality and HIPAA.

Salary: $10.00 - $13.00 per hour
