Job summary

OneWell Health Care is seeking an In-Home Caregiver in Summerville, SC to provide non-medical, in-home support. The role focuses on ADLs, companionship, light housekeeping, and wellness monitoring, with documentation and reporting of changes to the RN Case Manager.

Field-based work requires independent execution in home environments while adhering to care plans and regulatory standards. Required education is a High School Diploma or GED; prior caregiving experience is preferred.

Qualifications

• Provides non-clinical in-home support following care plans.
• Requires ability to observe, document, and report changes.
• Must maintain confidentiality and HIPAA compliance.

Responsibilities

• Assist with bathing, grooming, dressing, toileting, mobility, and transfers.
• Provide meal preparation based on dietary needs and hydration reminders.
• Perform light housekeeping and transportation when needed.
• Document daily logs and communicate with RN Case Manager and family.
