SYNERGY HomeCare of Southern Twin Cities is seeking a compassionate Caregiver to join our team in Burnsville, Minnesota. You will provide in-home care, including personal care, mobility assistance, meal support, and medication management to help clients maintain independence and quality of life.

As a Caregiver, you will offer companionship, ensure a safe living environment, and collaborate with healthcare professionals to deliver comprehensive care.

Responsibilities

• Provide personal care services, including bathing, grooming, and dressing, to our clients
• Assist with mobility, transfers, and ambulation as needed
• Prepare meals and snacks, as well as assist with feeding and hydration
• Manage medications and treatments according to client needs
• Offer companionship and emotional support to clients and their families
• Maintain a clean and safe living environment for clients
• Collaborate with other healthcare professionals to ensure comprehensive care
• Document client information and progress accurately and timely
