Job summary

Suncrest Companion Services, part of LHC Group, is hiring PRN Home-Based Caregivers to support clients in Folkston, GA and surrounding areas. You will assist with personal care, daily living activities, and accompany clients to appointments, while documenting care and maintaining safety.

We offer a comprehensive benefits package and opportunities for development. Qualified candidates have a valid driver’s license, reliable transportation, and CPR/AED certification preferred, with 6+ months home

Qualifications

• Must have a valid driver’s license, auto insurance, and reliable transportation or access to public transit.
• CPR/AED certification preferred or within 90 days of hire.
• 6+ months of home care experience preferred.
• Ability to work flexible hours.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medications).
• Support household tasks, meal prep, and accompany clients to appointments or errands.
• Monitor and document client condition, vitals, and incidents; maintain confidentiality; use EVV system.
