Job summary

Responsibilities

• Follow the individualized service plan as outlined with the agency and client.
• Develop and maintain rapport with clients and coworkers.
• Help the client maintain good personal hygiene.
• Maintain a healthy, safe home environment.
• Coordinate and prepare nutritious meals for the client.
• Engage with the client in a positive, enriching manner.
• Document care and duties per agency protocol.
• Communicate effectively with the agency team.
• Report changes in the client’s condition or home situation to the supervisor.
