Job summary

Centria Healthcare is hiring Certified Nursing Assistants and Home Health Aides to provide in-home support in Michigan. You will work one-on-one with clients in private homes, helping with daily activities and essential care tasks while following each client’s plan of care.

Qualifications

• CNA or HHA certification preferred or equivalent home care experience.
• Experience with fall-risk and mobility-impaired clients desired.
• Ability to assist with personal care tasks, documentation, and following care plans.
• Strong communication skills and reliability in a private home setting.

Responsibilities

• Assist with mobility and transfers to prevent falls.
• Provide basic personal care including hygiene, toileting, and dressing.
• Monitor oxygen therapy as needed and respond to changes in condition.
• Document and report changes in client condition promptly.
• Maintain a safe, clean, and healthy home environment.
• Follow care plans and local/state/federal requirements.
